Fortunately, each time the problem reappears, I am able to reapply your fix successfully which BTW I have saved on my hard drive to avoid searching for your fix each time. This fixed it for me. I searched for 2 days and I was nervous about downloading and installing a fix.
I still have no idea what the culprit was. The last change before "New Folder" disappeared was installing the latest iTunes, but not sure if that was coincidence.
